NEW REFEREES TO THE SITE
If you have never logged on to the District IV Website in the past, then you need to
register. You do this by going to and completing the Registration Form.

Fill in the needed information, including your address, date of birth is a must, in the
"notes" section write in that you are a U10 & Below soccer referee, and at the bottom
click the box next to "referee". When complete - click "Apply".
If you do not fill out the form properly and/or leave some mandatory data blank, you will
receive an error message back in Red at the top of the screen. Make the appropriate
corrections and click on Apply again.



Risk Management
All referees, no matter what age, are to submit for WSYSA Risk Management Clearance.
To apply for either an initial or renewal of an existing Risk Management clearance, we
recommend that you do so via your Club/Association Bonzi website if they have one. If
not, then please follow the instructions provided by WSYSA at this link -

Sort on 'Rules' for the type of document desired. Enter 'Addendum' in the search block for title. Item 12 should be: "D4 Operating Procedures Addendum - U10 & Below Rules"

Online RMA Application
The Washington Youth Soccer Risk Management Application must be filled out completely and properly submitted before any volunteer may begin working as a volunteer or employee of Washington Youth Soccer.

If you are registering for coaching and your club is on SAM please use the form provided by your CLUB (found on their website) instead of using this form. Registering at the club allows your club administrators to see the final status of your application. It also allows you to log in and update information (ie. Address, phone numbers, and email address). Volunteers, Referees, Vendors, and Club coaches not on SAM should use the following link:

Complete the form and check your email for a link that will inform you of the status of your RMA application.
The link will also tell you when your card was printed, provide your RMA number.


Youth Referees (those under age 18) are required to have on file the following documents:

Proof of Birth
Proof of Birth is usually a birth certificate, but other acceptable alternatives can work - such as a valid drivers license, a baptismal record or a hospital birth certificate.

Send the Proof of Birth document to the District following the directions below concerning Completed Forms/Documents

Parent/School Authorization document - This document must be re-submitted on an annual basis.

The required Parent/School Authorization form can be generated at Member: Paperwork-Authorization.
From this screen, simply enter the Name and Address information for the school you are attending, answer the question/s about other jobs and click on Display.

Next, print out the document and the Youth Referee must sign in the applicable block as the Minor Employee.
Next, have your parent complete and sign the Parent Authorization Section
Next, have a school official complete and sign the School Authorization Section
NOTE: If you are Home Schooled - annotate the Title block of the School Authorization Section with Home Schooled and then the parent should sign as the School Authority.

Send the completed Parent/School Authorization form to the District following the directions below concerning Completed Forms/Documents
